Fountain Park Assisted Living & Memory Care’s philosophy about aging is simple: getting older means getting better. Every day brings more experiences and insights. Every day adds value and worth to the lives of our residents.

ALZHEIMER’S AND DEMENTIA CARE

Our Memory Care Neighborhood provides our residents who have special needs with a safe, secure, stimulating and soothing environment. All of our employees in the Memory Care Neighborhood are specially trained to provide exceptional care for our residents with memory impairments and we have nurses available 24-hours a day. Upon admission and updated periodically, we will develop and implement an individualized care plan that addresses each resident’s unique needs. Our staff will engage our residents in every day activities and dedicated programming that they can feel pleasure and success in and which will help promote their self-esteem.

RECREATIONAL ACTIVITIES & SOCIAL PROGRAMS

There are always enjoyable things to do and places to visit when you are a resident at Fountain Park. Exercise classes, Wii games, entertainment, computer classes, contests and movies are just a few of the many activities our residents enjoy without leaving home. Families and friends are also invited to our Holiday celebrations. Field trips, shopping excursions and restaurant outings keep our Coach on the road each week.

MEDICATION MANAGEMENT

Sometimes just remembering to take medication is tricky, much less refilling prescriptions. Our staff takes the worry out of medication management. A pharmacy makes daily deliveries to Fountain Park and our staff assist residents with their medication needs.

HOUSEKEEPING & LAUNDRY

Fountain Park residents have places to be and things to do. Household chores are not among them. We provide fresh bedding and towels as well as laundry and housekeeping services.

SAFE & SECURE

From optimum design and materials to detection and alarms systems that prevent trouble, Fountain Park is worry-free. Round-the-clock professional staffing and emergency call systems give Fountain Park residents the peace of mind they deserve.

CASE MANAGEMENT

Among our many qualified staff is an individual who is responsible solely for assisting residents and their families with matters relating to their Fountain Park residency. Our Community Relations Director is a resource person and confidante who assists individuals in getting settled at Fountain Park, as well as with issues of transition or accessing community resources.

PERSONAL CARE

Our staff is never far away when you need a helping hand. Whether it is help with bathing or getting dressed, we’re ready to assist our residents with matters of a personal nature twenty-four hours a day.

WORSHIP SERVICES

Worship services are celebrated on-site by various denominations through the efforts of congregations in the surrounding community. Staying connected and faithful is part of our plan.

RESTAURANT-STYLE DINING

Three delicious meals are served waitress-style along with table-side serving in our spacious dining room. Each meal features a selective menu with multiple entrees. We also offer therapeutic diets for individuals with special nutritional needs. Evening snacks are also served daily.

Also available for our residents’ convenience and personal pleasure at a nominal fee are…

BEAUTY & BARBER SHOP

Look Sharp! Our on-site beauty and barber shop means you always look your best. You never know when that special someone will come calling!

CAFÉ & ICE CREAM PARLOR

Our café gives residents a place to mingle with friends and family, while enjoying tasty treats. Our ice cream parlor is open daily to residents and their families.

What is an assisted-living facility?

Our assisted-living facility is licensed by the Ohio Department of Health and operated for the purpose of providing long-term residential care, room, board (three meals a day and nutritious snacks), housekeeping, 24-hour personal care, a varied activity schedule, and 24-hour supervision. Staff is on duty 24 hours a day and personal care services, including medication assistance, are available at any time.

What are our rates?

We provide a unique rate structure in our assisted living community that allows residents to only pay for the services they need. Please contact us via phone at (419) 633-9191 or by using the contact form located on the top right side of the page to learn more about our specific rates, services, amenities and programs.

What is the criteria for living in this facility?

The Department of Health determines the criteria for entrance into assisted living communities. Residents may require some assistance with bathing, dressing, medication management and personal care, or they may be completely independent with these tasks. A resident who requires ongoing skilled services and care is not eligible for assisted living. The staff at Fountain Park as well as the resident’s primary care physician work together to determine eligibility. Please contact Fountain Park if you are interested in scheduling a preliminary eligibility assessment.

What is included in the price?

24-hour access to emergency call systems, case management, a full activity calendar, scheduled transportation, three meals a day, snacks, housekeeping, laundry, 24-hour supervision by trained staff, use of the facilities, and much more.

Is personal furniture permitted at Fountain Park?

Residents are encouraged to bring their personal furniture and other items to decorate their apartment. Many residents feel that these personal touches transform their new apartment into a comfortable and familiar environment just like home. Photographs, a special quilt, wall hangings and other belongings are welcome. Fountain Park can also furnish your apartment for you at no cost.

What happens when a resident needs a higher level of care?

If a higher level of care is required for residents in our assisted living community, we will work closely with the resident and their family to ensure suitable and appropriate placement.

Do we accept Medicaid or Medicare?

We do accept a limited amount of Medicaid residents based on availability and we do accept Long-Term Care Insurance. We also assist our residents/families in accessing Veterans Administration Aid and Attendance Benefits, however, please note that Medicare does not pay for assisted living. We have also partnered with several senior financial solution providers such as ElderLife Financial Services (which provides lines of credit to help seniors and their families pay for assisted living) and Life Care Funding Group (which helps fund assisted living using an in-force Life Insurance Policy.

Do we have transportation available?

Yes. We have scheduled transportation available for physician appointments and other outings. Our activities department frequently schedules our transportation for trips to local restaurants and community events.

What is the general community like?

Our residents here are active, some have walkers, others have canes, while some require none. We also have residents that still drive themselves.

Can the residents leave the facility at any given time?

Yes, residents are free to come and go as they please, as long as it is safe for them to do so. We ask that residents sign in and out at the front desk.

What appliances can residents have in their apartments?

A small refrigerator is allowed, however, we ask that you not keep blow dryers, hot rollers, popcorn poppers, space heaters, extension cords, heated blankets or cooking appliances (i.e. hot plates) in your apartment.
